The patient portal allows you to receive correspondence from your doctor or other professionals at Clinique médicale Le Sentier. It will also allow you to consult, download and print certain results that are shared with you by them.

CREATE YOUR PATIENT PORTAL ACCOUNT
- Have your health insurance card handy.
- Click Create Account.
- Fill in the fields that appear in the window, then click on Create an account.
- To complete the portal registration, send us an email containing your full name, date of birth, telephone number and email address info@cliniquemedicalelesentier.com. This activation step is essential to allow you to use your portal.
- Within 72 hours, we will process your portal membership request and you will receive an email to activate your portal account.
- You do not have to create a portal for children under the age of 14 followed at the Le Sentier Medical Clinic, but you can add them to your portal account. Indicate in the email to info@cliniquemedicalelesentier.com the name of the child as well as his date of birth.
